DPH Position Opening -Linkage to Care Case Coordinator

Job Summary: 

Experience: Typical candidate will have at least one year case management experience working with diverse populations around HIV, sexual health or other health navigation, or two years experience in another area of social services and case management. 

Knowledge, Skills & Abilities: Counseling skills. Knowledge of available community resources for medical, psychological, and social needs for people living with HIV. Knowledge of HIV transmission basics and care, preferably including a working knowledge of HIV laboratory values and medicines available to treat HIV infection. Basic competency in motivational interviewing. Competency in assessment of client’s medical, mental health and psychosocial needs. Ability to multi-task, strong organizational and communication skills. Microsoft Office.
 
 Hours: Monday-Friday 8:00am-5:00pm
 
 Location: Denver Public Health – HIV/STD Prevention 
 
Under general supervision, uses a client-centered or motivational approach to behavior change and proven intervention models to assist clients recently diagnosed or at risk for a chronic illness or clients with prior diagnosis and currently out of care. This position will focus on navigation and retention services for clients who are living with HIV or interested in HIV pre-exposure prophylaxis (PrEP).  Provides basic care coordination, identifies and reduces barriers to care, and provides referrals to necessary resources based upon individualized client needs and circumstances. Actively interacts with clients in community, clinic, or correctional care settings.

PrEP, HIV, PEP and Hep C Navigation
STD visits/ screening and HIV testing appointments
PrEP retention/reengagement/enrollment renewals
Medical chart entrees, data entry, tracking, reporting, in depth case coordination.
